The reason
Why people use it
- Persistent low energy, exercise intolerance and slow recovery lead people toward mitochondrial rather than hormonal explanations.
- MOTS-c is a mitochondria-derived peptide studied for metabolic and exercise-capacity effects, which makes it attractive as a non-hormonal metabolic lever.
- Elamipretide (SS-31) targets cardiolipin in the inner mitochondrial membrane and has reached genuine clinical trials in primary mitochondrial disease.
The evidence
What the research shows
- Elamipretide has the strongest position: multiple registered clinical trials in primary mitochondrial myopathy and Barth syndrome, with mixed primary endpoint results and continuing development.
- MOTS-c evidence is mostly animal and cell work reporting improved insulin sensitivity and exercise capacity. Human data are early and sparse.
- NAD+ precursor research in humans reliably raises blood NAD+ levels; whether that translates into clinical benefit in healthy adults is not established, and infused NAD+ itself has thinner evidence than the oral precursors.
- Fatigue is one of the most placebo-responsive outcomes in all of medicine, which makes uncontrolled reports here especially weak evidence.

How it is reported to be used
- NAD+ is most often reported as slow subcutaneous injection or infusion, because rapid administration is widely described as causing intense flushing and chest tightness.
- MOTS-c and SS-31 are reported in short cycles rather than continuously, generally alongside training.
- Reported effects are subjective and early — days to weeks — which is exactly where placebo response is strongest.

- Unexplained persistent fatigue warrants medical evaluation; thyroid disease, anaemia, sleep apnoea and depression are common and treatable.
- Rapid NAD+ administration is consistently reported as acutely unpleasant and is not a benign procedure.
- None of these is approved for general wellness use.
