The Placebo Problem in Wellness Research

In studies of energy, mood, recovery and libido, placebo arms routinely improve. Any claim built on an uncontrolled trial is uninterpretable.

Placebo response is not imagination. It is a measurable, reproducible change produced by expectation, attention, ritual and regression to the mean — and it is largest precisely for the outcomes wellness products target.

This is why a before-and-after study of a subjective outcome carries almost no evidential weight, no matter how many participants it includes.

Regression to the mean

People start interventions when they feel worst. Symptoms fluctuate. A proportion of any improvement is simply the natural return toward an individual's average state.

Only a control group that also enrolled at its worst can separate that from a treatment effect.

Blinding is fragile

Injections sting, some compounds flush the skin, and side effects can unblind a trial in the first week. Good studies report whether participants correctly guessed their allocation; most do not.

When a compound has an obvious immediate sensation, treat any subjective endpoint in that trial with additional caution.
