The discovery that mitochondrial DNA encodes signalling peptides, not just respiratory chain components, opened a genuinely new area of physiology. MOTS-c and humanin are the best-known examples.

This field is worth following precisely because it is early. Human data are mostly observational: levels rise with exercise, differ with age, and track metabolic status.

Cardiolipin and clinical trials

Elamipretide, the clinical development name for SS-31, has been through registered randomised trials in rare mitochondrial disease. Results have been mixed, with primary endpoints not consistently met.

Mixed trial results are not a scandal. They are what the process looks like when a mechanism is real but the clinical effect is uncertain.
