Human lifespan studies are structurally difficult: they take decades, cannot be blinded indefinitely, and require enormous samples. This is why the longevity field relies on surrogate markers.

Surrogates are legitimate research tools and poor marketing evidence. A change in a biomarker is a hypothesis about outcomes, not a demonstration of them.

Epitalon as a case study

Epitalon's reputation rests on a regional research tradition with limited independent replication and reporting that cannot be appraised against modern standards. That earns grade E — not because the idea is absurd, but because the evidence is not there.
